For the 1099-SA form, who is the Payer, is it the employer or the institution who held the HSA?

Typically it's the bank, plan administrator or the institution who held the HSA.  You are the payee, and your employer shouldn't appear on the form at all.

Typically it's the bank, plan administrator or the institution who held the HSA.  You are the payee, and your employer shouldn't appear on the form at all.
