For medical bills can I deduct before insurance has paid out or what is left to pay after insurance pays

Only what you actually paid yourself out of pocket.  Not what was billed or what the ins paid.

You can deduct only the amount that you actually paid out of your pocket. What was billed does not matter, what insurance paid does not matter. The only thing that matters is what you actually paid out of your pocket.
