Filing 2017 Ga tax. In Contributions to a HDHP section it is asking me to enter 100% of annual premium. Is this the deductible I paid to the providers?

No.  The premium is the monthly amount you paid to have your insurance.

So does that mean I would include the premiums I pay my employer for medical, vision, and dental for the year?

It would only be the medical.  But exercise caution, because the GA deduction is based on premiums you pay with after-tax dollars.  Since you say you were paying your employer, it is likely that the premiums were pre-tax and do not qualify for the deduction.  If they are pre-tax, the deduction is reflected in your taxable income already.
