First, there are no reporting requirements for an FSA. It does not appear on your tax return.
HOWEVER, you are not permitted to contribute to an HSA if you are covered by an FSA. In the HSA interview, you will have to indicate that you are not covered by an HDHP policy for the months that you were covered by the FSA. This will cause most or all of your HSA contributions to be declared to be in excess, causing all sorts of difficulties.
Please come back and tell us how you were covered by an FSA (was it your spouse's), for what months, and for what months you were covered by Family or Self-only HDHP.
