Contributions that you made via payroll deduction will be reported on your W-2, and you can’t file your tax return until you receive your W-2.

However, the banks usually do not issue this form until May, and you do not have to wait to receive this form before you file your tax return. File your tax return using your own independent records of what you contributed.
make sure you do not confuse the two. All contributions made via payroll deduction are considered “employer” contributions and are only reported on your W-2 even if that includes your contributions through a voluntary salary reduction. Your voluntary salary reduction should not be reported as an out-of-pocket contribution later in the program.

The answers by both Champs above are correct. Since you likely contributed to your HSA through your employer, the form you would need is your W-2, as noted above. Please do not try to file without your W-2 - e-filing to the IRS does not open for a couple of weeks anyway, so there isn't any point in doing it now.
NOTE: many HSA custodians (the bank or financial institution who has your HSA) make any HSA forms available online, so you might go visit the custodian' website and see what's there. Of course, there probably isn't anything there for contributions now (they will be on your W-2, or your 5498-SA which as was pointed out, won't come out until much later). However, this may prove useful if you spent any dollars out of the HSA< because this will be reported on a 1099-SA, and you may see this on the website much sooner than it comes in the mail. Yes, if you spent money out of the HSA, please wait until you get the 1099-SA before filing (it should be mailed by the end of January).
