Contribution to HSA based on last month rule?

I started my job in July of 2016 and my HSA started shortly after. I have $500 annually deducted from payroll. On my W2, Box 12...I have code W with an amount of $1000.04. I believe my employer contributed the other $500. I did not make any other contributions and this is the first HSA I have ever had. Do I answer "Yes, a contribution based on the last-month rule was made." or do I answer "No."?

If I answer "Yes", what amount would I put in the "Excess Contribution Amount Using the Last-Month Rule" box?

Because you did not have a HSA last year (2015), you did not use the last month rule. Answer NO.
