Codes for box 12 for medical ins of 2

i have 2 different numbers in box 12. There is a W associates with the first and a D associated with the second.Which one do they want?

Enter both letter codes on separate lines. Select the '+Add another row' link to for the second code. Code W is for HSA accounts and code D is for elective deferrals to retirement accounts.
