Can I deduct health insurance premiums as medical expenses if I used an employer health plan but paid all premiums with after-tax dollars?

I took a buyout from my company in December 2016 but was eligible for 12 months of extended medical coverage. I paid the premiums with after-tax money; nothing was deducted from my severance pay or final wages.

Yes, so long as the health insurance premiums for paid for with after-tax dollars, you can deduct them on Schedule A (Itemized Deductions). See Federal->Deductions & Credits->Medical->Medical Expenses.
