Then is there some extra W2 pages listing the 4 states?
Yes, there is three W2"s. in box 15 CO, TX, NM, and ND are stated. In one of the W2, box 15 is MANY, box 16 has the same amount as box 1. box 17 has an amount
Ignore the first box 15 that says MANY. Then enter the other 3 on the same W2 screen.
If the EIN is the same and you work in more than 1 state or city, sometimes they will give you another W2 with the extra state information on it and the top part the same or not filled in. If this is the case, enter 1 W-2 but include all state and local taxes on it. It will let you type in more than one line for boxes 15-20. Don’t add the boxes together.
That response did cross my mind but this is a first I have seen this happen on a W2.
