What documents are required when mailing North Carolina state tax returns? Do I also need to include 1040 with my state tax return forms?

What are the document list when filing the NC state tax. I remember in the past, Turbo tax used to give a list of documents. Now it just says "your tax return" in the state filling instruction. So do I still need to attach 1040?
