What do I do if the employer state id isn't on the tax return?

I recieved a w2 and 1099 misc because of a settlement. And the w2 is missing the employer state id number. Who do I contact?

Enter the federal EIN in the state EIN box. If you are in one of the states below with no income tax, boxes 15-17 will be blank.

Alaska, Florida, Nevada, South Dakota, Tennessee, Texas, Washington and Wyoming
