leave 15-17 blank because the state does not have a personal income tax

Tennessee does not have a state income taxes.  Leave boxes 15 thru 20 blank on the TurboTax W-2

That box is used by your employer for various reasons.  You will have to ask your employer.

In the dropdown for box 14 scroll down to the bottom and select Other (unclassified)
