I live in and work in New York State. I hold clinical licenses in Connecticut and Pennsylvania and a telehealth practicing certificate in Florida. I see the clients in these three state states via telehealth only. I am a sole proprietor so I don’t have a registered business. Do I just file my taxes in New York? I will get 1099s because I contract with companies.

I think, if those companies have offices in PA and CT you might be required to file state taxes, even if you work remotely, especially if you're a subcontractor and they'll send you 1099 forms. I know NY requires remote workers to pay taxes in NYS if a company maintains an office in NY.

A. Yes, and only in NY, since you do not ever physically work in any of the other states.
NY and PA (and CT, in some circumstances) are among a handful of states that tax telecommuters. But, you do not meet any of the circumstances for that. California, to my knowledge, is the only state that taxes out of state self employed persons, with customers in CA. Even that would not apply, in your case, since your clients are individuals, not businesses.
