I received a letter from Department of Revenue Requesting Additional documentation .
They want me to send them copy of my Form w-2 as proof of withholding.. It also says include a copy of this letter, with the amount of your requested refund, daytime phone number and signature.

At anytime the fed/state can request anything needed to process your return. It is possible your employer did not send the W-3/2 at all or on time and/or the state didn't get their copy yet. So simply comply as requested ASAP and/or call the number on the notice if you have more questions.
It's also possible that your employer failed to remit the taxes withheld to the State.
