Sate Tax Form - issue with the first page

Hello

I prepared my taxes with TurboTax however while getting the ready to print (I have to manually mail my taxes) I see that the first page of my state tax is blank on the first half (instead of the blocks) is blank with my name and details. My name isn't fully correct hence I'm not sure if I can file it as I'm not allowed to send my fed taxes with the state taxes this time. I'm unable to get in touch with support no matter how much I try.

IS there anyone that can help me?

What state?  I think California is like that.

Yes, that's right. It's California. I wanted to fill in the first page seperately and pin it with the rest of the forms, however there is a change in the numbering(bottom most digits of the page) and it has 2 more forms attached which I'm unable to find online. 

Hence I'm unsure on what exactly to do.

Not sure I understand what you are trying to do.  It didn't print all the pages?  The printouts from tax programs may not match the blank forms from CA.  They can print different ways maybe depending on if you are getting a refund or tax due and they go to different addresses.  

Here is a blank 540.  I guess you can hand copy it to all the pages.

Is it okay for me just refill the tax forms and send it? I see that the serial numbers on the bottom on the forms that you just sent and the ones TurboTax prepared are different (as I have attached) 

I also have a Wage and Tax Statement form and California Adjustment - Resident form form attached. Will it be alright if I just replaced the CA540?
