On my form 1099-R Box 12 has exempt in it which means this income is exempt from state taxes. Where do I enter this information?

Leave boxes 12-14 blank.  They are trying to tell you that they did not withhold any state tax, but violated IRS rules by putting it in a box that only allows a dollar amount and only if state tax was withheld.
