NJ-2440 Pandemic Sick Leave

I received Pandemic Sick Leave in Sept 2021 as noted on Box 14 of my W-2 . Do I exclude this from my New Jersey income on form NJ-2440?

No, not if it was FRCCA sick leave- Form NJ-2440 is for Exclusion for Amounts Received Under Accident and Health Insurance Plan for Personal Injuries or Sickness.  The pandemic sick leave was paid by your employer not insurance.
