MY 2016 W2 HAS PENNSYLVANIA STATE, LOCAL & UNEMPLOYMENT TAKEN OUT & I HAVEN'T LIVED THERE SINCE AUGUST OF 2015

You will have to file a Pennsylvania non-resident return in order to get a refund of the tax that was withheld.  If you still work for that employer, you should also contact your employer right away and make sure they correct the withholding for 2017.
