Live in OH but work in KY. W-2 saids Ohio state withheld. I also pay Kentucky city and county taxes. Do I put I made income in different state and file non-resident KY?

Line 15- W-2 saids Ohio withheld, but box 18-20 have Kentucky taxes withheld. Do I need to file any Kentucky forms?

No, you do not need to file any Kentucky forms. Ohio and Kentucky have a reciprocal agreement and your employer correctly withheld state income taxes for your resident state. They also withheld a small amount for their locality. 

 

If you are required to file an Ohio school district tax form, you may receive a credit for the taxes paid to the Kentucky locality by your employer. You would have to check with your particular district to see if that is an allowable credit.
