Hi Tony, Sorry for the slight delay but I am back on it. The error looks to be the entry for the Nationwide Life Insurance 1099-INT. It is showing a $1 withholding for Massachusetts but the missing field is the state ID number. The data shows it was imported from Quicken or Quickbooks. Do you have the actual 1099-INT issued? Please check if there is an entry for Box 4. Thanks. @tonyjag

The NATIONWIDE INSURANCE 1099-K is hard copy only and the numbers were hand-entered by me via Turbotax.

Box 4 is blank. Here are the relevant populated boxes

All other numbered boxes are blank
The $1 is probably the result of trying to get Turbotax to allow us to efile. As i recall, it would not accept 0 so i entered $1.

Can you go into the Forms (click the Forms icon located in the upper-left corner of the screen) and change the Box 16? @tonyjag
OK, you mean form Schedule 62-WH? it has no box 16
No, look for the 1099-INT in the list in the federal section- you will be able to edit the state ID number in Box 16. @tonyjag
OK, but box 16 already contains 3 1 4 1 5 6 8 3 0 in light blue. Box 17 contains $1 in darker blue.
