This didn't show the majority of my question...which was: I'm filling out the 'State Residency Information' section. I lived in Illinois from January-November and lived in Indiana from November-Present. I work in Illinois. Should I check yes to Filing A Return in Illinois, Indiana, or both?
Both since you resided in both. working the whole year in Illinois makes all your wages/ self-employment income taxable in Illinois. any other income is taxable only if earned while a resident. In addition, working in Illinois while residing in Indiana makes the income earned in Illinois while residing there taxable along with any other income earned while residing in Indiana. you get a credit on Indiana for some or all of the income taxes paid to Illinois for income earned there while you resided in Indiana
