I live in AR and work from home for a company in LA. My state withholding is withheld in LA. Do I need to include my income on the LA or the AR form?

You should only be paying taxes to the state in which you earned the income: AR. The company should not have withheld taxes for LA, but the only way that you can get the withholding back is to file a tax return for LA, and show that none of your income is taxable in that state. 

The problem is that Arizona will tax you on the income that you made in that state, and if you had no withholding during the year you will wind up with a balance due.

If this is your current situation, you should have your employer's  payroll department change your withholding to the correct state.
