I have 2 W-2 forms for having worked in MA state for ~50 days and the remainder in NY state, which form do i use to get the "Income & Taxes withheld" information?

You would use ALL the Federal with holding you can find -- All your W-2's and 1099's ...  Any Federal withheld tax will lower your final tax owed.
