I don't understand what the PA "Schedule EU" is. It's asking me to fill in employer address info, but I had multiple employers in PA. The form is not specific.

Schedule UE is for unreimbursed business expenses.  You must file a separate Schedule UE for each employer where you had allowed expenses but were not reimbursed.  PA allows such items as union dues, uniforms, small tools, and business (not commuting) miles. 

PA does not allow mileage between multiple jobs or deduction for trade publications or professional societies unless it a requirement for employment or by law  (licenses and continuing education required to keep job or by law).

PA law does not follow federal law so answer questions carefully.
