I called the IRS and got very simplified version of a W2. It does not have anything for state tax withheld so does that mean there was no state tax withheld?

The IRS Wage and Income transcript only shows the amount withheld for federal taxes--it does not show state information. You need to ask your employer for another copy of your W-2 if you do not have your state information. Going forward--ALWAYS save your W-2 for your own records.
