How is the Indiana 529 contribution identified on the form?

There is no federal deduction for making a contribution to a 529 plan. Some states allow a deduction on the state return. If your state is one, it will come up in the state interview.

How is the Indiana 529 contribution identified on the form?

According to the instructions for the Indiana income tax return, credit for contributions to the Indiana 529 plan are initially reported on Line 6 of Schedule 6 Offset Credits.  It will be shown with code 837.  Then this total amount goes to Line 13 of Form IT-40.
