This is asking if you want the IRS (or state) to keep your tax refund and apply it to what you might owe next year.  

If you want your refund deposited now (or mailed), your answer to this question should be no.

Let me know in comment if that does not clarify this for you.

Do you have to do anything to claim the amount you pushed forward to the following year, if you didn't owe?

Yes.  Next year you need to enter the amount applied from the prior year as an estimated payment.
