I retired from the Federal Government in 2021 and moved to NC from Maryland in late 2021. I completed for DS-5014 with HR. I thought this would change my withholding STATE, but it didn’t.
I alerted my tax preparer and she said when my taxes are filed with NEW address, IRS would immediately update my STATE status. But, this did not happen ~ withholdings are still be taken from Maryland. I contacted my payroll off and was told to contact IRS. It took forever to reach a human on the phone, but when I did she sent me Form NC-4. Which my tax preparer advised me is the INCORRECT form.
Please advise how can change the STATE for my tax withholdings. Thank you!

@DMHarley MARYLAND TAX WITHHELD IN ERROR
If Maryland tax was withheld from your income, you must
file to obtain a refund of the withholding. Complete all of the
information at the top of the non-resident form through the filing status,
residence information and exemption areas. Check the box
provided to the right of the residence information for you
to indicate your withholding was withheld in error. Enter
your federal adjusted gross income on line 17 in both columns 1
and 3 and line 24. Then complete lines 43-47, 49 and 51.
Sign the return and attach withholding statements (Forms W-2
and/or 1099) showing the Maryland tax withheld equal to the
refund you are claiming.
Your return is then complete. You must file within three years of
the original due date to receive any refund.

DO NOTE that Maryland if your home is outside
Maryland but you maintained a place of abode (that is a place
to live) in Maryland for more than six (6) months (183 days) of
the tax year and you are physically present in Maryland for 183
days or more (you are a statutory resident).
You are a part-year resident if you established or abandoned
legal residence during the tax year.

Who is the Payor on your 1099-R?
That's who you should contact about State Withholding.
Many retired people don't have state tax withheld from their retirement income, for several reasons. Many states exempt retirement income, so it's not taxable anyway, and many retired folks move from state to state and don't want to run into the same problem you are having with changing state withholding.
@DMHarley Definitely you should check with the Administrator or Agents handling that Retirment fund.
For some reason, that DS-5014 didn't get implemented, and perhaps you need to send them a new one.
Also, if you have Online access to your retirement account there, sometimes you can make the change in your account yourself.
