If state taxes were withheld from your annuity, you will need to report them when you enter Form 1099-R, in Box 14.

TurboTax will prompt you in case you need to enter state income if you report state taxes withheld.

If you don't owe state taxes in the state for which tax was withheld, because you don't have enough income to exceed the filing requirement, you can still file a return for that state in order to get the withheld tax refunded.

See this article for more information on Form 1099-R.

See this help article for more information on whether you need to file a state return.

If Wisconsin is your resident state, the state income would be the taxable amount (same as federal).  When you complete the Wisconsin state return, any adjustments will be made and the withholding applied to pay any tax liability. If the state withholding is not from your resident state, you would enter 0.  In TurboTax for entry purposes, the state income is needed to transfer to the state return.  @42Milestone
