Arizona - other income (from HSA distribution)

Hello there,

On AZ form 140PY, line 22, I am reporting "other income" from an HSA distribution. The actual line reads "22 Other income reported on your federal return: Include your own schedule"

What does "Include your own schedule" mean? Should I attach the Sch 1 from1040 or 8889 to the state return?

No, the program generates a statement to go with the return. A statement is just what is sounds like, a paper with a description, like HSA and an amount.
