I have a 1099-MISC from a medical fund annuity, does this have to be claimed as self-employed income?

No, that would not represent self-employment income. After you enter in the Form 1099-MISC, make sure you indicate that the income is not related to your job, you only receive it in the current year and you did not have a profit motive for it when asked. That way the income will not show as self-employment income.
