Contact the Texas Lottery at &lt;A href="http://www.txlottery.org" rel="nofollow" target="_blank"&gt;www.txlottery.org&lt;/A&gt;&amp;nbsp;or&amp;nbsp;(800) 375-6886, for a copy of the form.&lt;/P&gt;&lt;P&gt;While you wait on the copy, you can enter the approximate winnings without the form. &lt;B&gt;'Skip W-2G' &lt;/B&gt;is an option when you first navigate to the section. If you have already brought the form up, continue until you reach the summary page and delete.&lt;/P&gt;&lt;P&gt;
