      <description>&lt;P&gt;There are many types of 1099 forms. The most common are:&lt;/P&gt;

&lt;UL&gt;
 &lt;LI&gt;1099-MISC - you can contact the contractor and request a new copy.&lt;/LI&gt;
 &lt;LI&gt;1099-R - This is for retirement withdrawals, and you can contact the company&lt;/LI&gt;
 &lt;LI&gt;SSA-1099 - you can look this up online&lt;/LI&gt;
 &lt;LI&gt;1099-Div or 1099 INT - contact the Bank or Broker&lt;/LI&gt;
 &lt;LI&gt;1099-B - is stock transactions, contact your Broker&lt;/LI&gt;
&lt;/UL&gt;

&lt;P&gt;As you can see from all of these you should go back to the source.&amp;nbsp; Contact the person or company that issued it.&amp;nbsp; Many&amp;nbsp;times these forms are online.&lt;/P&gt;</description>
