  &lt;B&gt;You must have a 1099R and sometimes when people are laid off they are asked if they want to take the amount in the retirement plan out. &amp;nbsp;&lt;/B&gt;
&lt;/P&gt;&lt;P&gt;
  &lt;B&gt;If you believe the 1099R is incorrect, you need to call the prior employer or whoever issued you the 1099R.&lt;/B&gt;

      <description>If your 401k was small say a few thousand, the employer may close it out without asking you. You would get a separate check. You would have had 60 days to move it into another qualified plan.</description>
