This is the one that you need to report your Uber earnings. I know this because I to drive for Uber. Another piece of advice, the earnings under your 1099 is incorrect, and you need to add an expense to your return that subtracts the total on your 1099k from the fees that Uber took out of your income to equal your actual profit, and dont forget to deduct all your expenses so that will put more $$$ back into your pocket.
