If you have a 1099-R, you would just need to enter the form exactly as it appears. If you are having trouble with a subsequent question, copy the question from the screen and let us know what it is referring to.

Everything is entered as per on the 1099R but in addition to that the Turbo Tax program is asking "Where is this Distribution from" I narrowed it down to 2 answers (1) Not eligible for exclusion for those over age 59 1/2 and

(2) None of the above ( Select for deferred compensation 457 plans) . I'm not sure it I should use #1

Did you enter the PAYER’S name, street address, city or town, state or province, country, ZIP or foreign postal code, and phone which is found to the left of Box 1?
