I am going to assume that you meant "HSA (Health Savings Account)" instead of "hsc".

Form 5329 reports on penalties imposed for a variety of situations: excess IRA contributions, excess HSA contributions, early IRA distributions, etc.

While excess HSA contributions certainly are a common cause for a form 5329, there are many others. You will have to print the form or view it as a PDF to see which lines on the 5329 are completed, so that you can know why the form is there.
