    <description>&lt;P&gt;Unless there is a code "B" in box 7, box 11 should be blank.&amp;nbsp; If there is a code "B" then contact the 1099-R issuer.&lt;BR /&gt;&lt;/P&gt;&lt;P&gt;Boxes 12-14 are for state tax withholding.&amp;nbsp;&amp;nbsp; If box 12 is blank or zero on your 1099-R then 12-14 should be blank - not zero.&amp;nbsp;&amp;nbsp; If there is state tax withholding (rare for a Roth), then box 13 must have the state ID and the payers stat ID number.&lt;BR /&gt;&lt;/P&gt;</description>
