You need to edit any actual IRA 1099-R forms, and make sure the box near box 7 has been checked. The box is labelled IRA/SEP/SIMPLE. Only check thatt for IRAs, not for Pensions nor 401k/403b/457/TSP type accounts.

If the payer did not mark the IRA/SEP/SIMPLE box and box 7 does not have code J, T or Q, the distribution is not from an IRA and the distributions must instead appear on lines 5a/b.

Are you sure the 1099-R is actually from an IRA account, and not some other kind of retirement account?
