I see this very problem addressed last year, but it appears to be happening again: I have deleted and resubmitted my 1099 – R several times, but it still appears to be triggering a request for a Canadian Institution name. Looks like this was a glitch last year. Anyone else having the same problems and do we know if there is a fix?
