      <description>&lt;P&gt;I cashed out my 401k after leaving the company I worked at for nearly 5 years.&lt;/P&gt;&lt;P&gt;Why did I receive 2 1099 Forms in the mail?&lt;/P&gt;&lt;P&gt;It is showing tax withheld on the 1099 form with the larger amount listed, yet showing all zeros on the form with the smaller amount.&lt;/P&gt;&lt;P&gt;I'm confused because I only received the one time payout of the larger amount.&lt;/P&gt;&lt;P&gt;Any idea why I received 2 forms? And should I include both of them?&lt;/P&gt;</description>

      <description>&lt;P&gt;You should contact the 1099-R issuer for additional information.&lt;/P&gt;
&lt;P&gt;One might be a correction of the other, or the disbursement method might have different categories. You might also have had different accounts within the same company. This is just general information.&lt;/P&gt;
