      <description>&lt;P&gt;You should enter your &lt;B&gt;&lt;I&gt;CSA 1099-R&lt;/I&gt;&lt;/B&gt; exactly as shown on the form.&amp;nbsp; The IRS has a copy of it.&amp;nbsp;&lt;/P&gt;&lt;P&gt;If there is &lt;I&gt;no amount in Box 5&lt;/I&gt;, that means that no part of your pension was used to pay &lt;I&gt;Health Insurance&lt;/I&gt;.&lt;/P&gt;&lt;P&gt;Any amount in Box 5 is transferred to &lt;I&gt;Schedule A, Itemized Deductions,&lt;/I&gt; for &lt;I&gt;Medical Expenses.&lt;/I&gt;&lt;/P&gt;&lt;P&gt;If you paid your own &lt;I&gt;Medical Insurance Premiums&lt;/I&gt;, you can enter this amount yourself in the &lt;B&gt;&lt;I&gt;Deductions &amp;amp; Credits &lt;/I&gt;&lt;/B&gt;section.&lt;/P&gt;&lt;P&gt;Here's more info:&lt;/P&gt;&lt;P&gt;
