Who should receive the 1099 R form? my daughter as an alternate payee (as my ex spouse was deceased and per an amended DRO she received lump sum payment that she cashed out and taxes were withheld. The 1099 R form came to me with code 7 checked. If this is correct how does she report the money she received on her taxes.

If your daughter was the payee, then the 1099-R should go to your daughter. You will need to contact the plan administrator to get that corrected.
