Which form you need depends on why you are getting $272 a month, and what kind of account it comes from. Click this link to view your tax forms in your Fidelity online account. The form you need will be there.

Or do the following.

Log in to your Fidelity account.

Under Accounts & Trade click Tax Forms & Information.

On the Tax Information page, click the button that says "View your tax forms."

If you have a pension, annuity, IRA, or other type of retirement account, Fidelity should have sent you Form 1099-R for any distributions you may have received. Where do I enter a 1099-R?
