You can only enter 4 lines in box 12 before you need a second w2. See here.

I'm creating W-2's for a business in Intuit Quick Employer Forms and I need to use code BB in box 12 for Roth contributions to a 403(b) plan. However there is no option for code BB in the program. How do I report these contributions on the W-2 or better yet, how do I get code BB to populate? I'd appreciate any help...

It depends. Have you scrolled through the entire drop-down to view all the distribution codes available?
