Which option for "Where is your distribution from?" do I select, option 4 or option 5?

I have entered my IRA 1099-R info and Turbo Tax asks where is the distribution from. I know it is none of the first 3 option so how to determine whether I should select option 4 or 5?

Please clarify ‌the title of the screen that you are on where you see the options.

The options presented vary widely, depending on which state you will be filing for.
