Box 11 on the 1099-Div is in a strange place. It is in the middle left of the form next to Box 12. It is a simple check box.

My 1099-DIV form is on a 1099 Consolidated Tax Statement and does not have a box 11 within the 1099-DIV section. Where should I look for this to answer the FATCO question?

No, don't check the box for FATCA if it doesn't show on your Consolidated 1099 statement.

By law, your financial institution is required to give you all of the information that you need to file your tax return, so if the box isn't even there, don't check it in TurboTax.
