If there is no code in box 7 of the Form 1099-R then you will have to contact the plan administrator for a corrected Form 1099-R. The distribution cannot be reported on a tax return without a code in box 7 of the 1099-R.

A common code for box 7 is a 7. You might miss it because it is the same as the box #. Or sometimes they write Normal or non disability in it. Those are both code 7. Check with the plan.
