Cost of ambulance will go to medical expenses, the same category as doctors and dentists.

The cost of the ambulance is a medical expense deduction if you itemize and it was not reimbursed.

The TT Medical expense categories are just for your use, as only the total ends up on your tax return. So you can use any of them. I personally keep a spreadsheet all year, and then just enter the overall sum into TT.
