&lt;LI&gt;Check "&lt;B&gt;1099-R withdrawal of money from 401(k) or other retirement plan"&lt;/B&gt;&amp;nbsp;&lt;I&gt;and&lt;/I&gt;&amp;nbsp;check the box at the bottom &lt;B&gt;"I need to prepare a substitute 1099-R"&lt;/B&gt;
